• El presente formulario constituye un instrumento de solicitud de subsidio. El llenado del mismo no compromete a la Fundación Pro Niños y Jóvenes La Esperanza a otorgarlo.

• Los subsidios son otorgados tomando en cuenta exclusivamente los criterios de depuración de la fundación, según la disponibilidad de fondos.

• Los datos proporcionados en este formulario deben ser reales y quien completa el mismo autoriza a la Fundación Pro Niños y Jóvenes La Esperanza a comprobarlos.

• Los estudiantes de familias beneficiadas con subsidios escolares deben cumplir con los requisitos académicos y de comportamiento exigidos por el centro educativo, de lo contrario, la ayuda es retirada de inmediato.

• El subsidio solicitado consiste en un descuento voluntario que la Fundación Pro Niños y Jóvenes la Esperanza brinda para ayudar a familias necesitadas. Si luego de obtener el descuento el tutor muestra evidencias de poseer un ingreso económico satisfactorio, deberá renunciar al mismo para que este sea otorgado a otra familia.

• Adjunto a este documento, el responsable debe enviar las calificaciones recientes del (los) estudiantes.
